Free Lasik or Conductive Keratoplasty (CK) Contest Rules

Contest open to legal residents of Pennsylvania and New Jersey, 18 years of age or older.  Entry limited to one per contestant, and must be sent by postal mail.  Incomplete entries will be void.

Winner shall be required to enter into a physician/patient relationship with Lehigh Valley Eye Center, P.C. .  Winner is required to have a preliminary eye examination at Lehigh Valley Eye Center, P.C., to determine eligibility for LASIK or CK.  Should any winner prove to be ineligible for LASIK or CK, an alternate winner will be drawn at random. 

The last date entry forms will be accepted is August 14, 2009.  Drawing will take place August 21, 2009.  Winner will be notified via telephone or e-mail.  Odds of winning depend on number of entries received. Prizes are not transferable.  No substitutions or exchanges allowed.  Procedure must be performed within three (3) months of drawing. Surgery will be done by Dr. Leahey.

Entry constitutes permission to use winner's entry, name, hometown, voice, likeness, photograph and any statements regarding this sweepstakes for editorial, public relations, promotional and advertising purposes on behalf of Lehigh Valley Eye Center, P.C. without compensation, unless prohibited by law (“Publicity”).  Winner will be required to sign an Eligibility and Release of Publicity Form within 10 days of notification or an alternate winner will be selected by a random drawing.    Subject to all U. S. federal, state and local laws and regulations.  Void where prohibited.  Taxes on prize are the sole responsibility of the winner.
